Intelligent electrodes
Form comes with two custom-made electrodes designed to provide optimal muscle coverage. The small and large electrodes allow for use on muscles of different sizes.
The "intelligent" electrodes automatically configure the program parameters depending on the selected muscle group.
When used correctly, the electrodes maximize motor point activation to trigger small muscle contractions, resulting in effective muscle recovery in just 20 minutes.
Our innovative electrodes use reusable and replaceable hydrogels that provide between 10 and 20 uses before needing to be replaced, minimizing waste and maximizing effectiveness.

Contraindications
• Do not use this device if you have a pacemaker (or if you have heart rhythm problems) or with other electronic medical devices. Using this device with electronic medical devices may cause the device to malfunction. Electrical stimulation in close proximity to an implanted device may affect some models.
• Do not use EMS during the first three months of pregnancy. It is unknown whether EMS can affect fetal development.
• Do not use EMS on your abdomen during later stages of pregnancy. Discontinue use immediately if you experience any unexpected side effects.

Q: Why won't the display turn on?
A: The battery may be dead. Try charging your Form. If that doesn't work, contact us.
Q: Why doesn't the stimulator connect to the electrode?
A: The stimulator is in the wrong orientation. Rotate the stimulator so that the shapes on the bottom of the stimulator and the top of the electrode align. This should attract the magnets in the stimulator and electrode. See section 4 of your user manual.

Q: Why is there no sensation?
A: Make sure your device is charged. The intensity might not be strong enough; try increasing the intensity. There might be a bad connection between the electrode, stimulator, and body. Make sure the electrode is securely attached to the body. Ensure the connection between the stimulator and electrode is secure. If the electrodes don't easily adhere to the body during a full session, you may be using old hydrogel pads and need to replace them.
Q: Why is the display flashing?
A: This can happen for three reasons: Low battery - make sure your Form is charged. Connection error between the stimulator and electrode - unplug and reconnect the stimulator and increase the intensity. Connection error between the electrode and body - make sure the electrodes are properly attached and replace the hydrogel pads if necessary.
Q: Why can't I change the intensity / why doesn't the dial work?
A: The stimulator is locked. Unlock the stimulator by pressing the control button twice.
