The Primo Combination 860 is an ultrasound/EMS combination device with a multi-frequency ultrasound head (1 and 3 MHz) and a wide range of currents. The two therapy modalities can be used separately or in combination. The large touchscreen display allows for convenient selection of the various programs using a body atlas.

The combination of 2- and 4-pole interference therapy with ultrasound is particularly effective, but various other waveforms are also available, including diadynamic currents, TENS, sinusoidal, galvanic and interrupted galvanic, faradic, Trabert, Medi-Wave, Russian and microcurrents.

This wide range of therapeutic currents makes the device highly versatile. These include acute and subacute, traumatic and inflammatory conditions, chronic arthritic and rheumatic conditions, and general applications for pain therapy.

Ultrasound dose setting

A truly interesting feature is the calculation of the ultrasound dose to be delivered in a single treatment session. After entering parameters such as lesion depth, tissue status, and the size of the treatment area, the maximum dose to be delivered is calculated.

When this maximum value is reached, the ultrasound treatment is automatically stopped.

The following is a description of the available waveforms and the respective display views (the display language can also be set to German):

Interference 4-pole

Interference currents
Treatment time: Selected by touching the digits on the time display or by tapping the clock icon and entering the desired treatment time. The time can be set in 30-second intervals.

Carrier Frequency: Touch this button to select the carrier frequency: 4, 8, or 2 kHz. AMF (Amplitude Modulation Frequency): The fundamental and peak frequencies can be adjusted from 0 to 250 Hz in 1 Hz increments.
Constant Current / Constant Voltage: The device's output can be set to constant current or constant voltage. In constant current mode, the electrode impedance is monitored. If the impedance is too high for any channel, stimulation on that channel is terminated, a "Lead Error" message is displayed, and an alarm sounds.

If the electrode impedance increases in constant voltage mode, the current is automatically reduced. A constant voltage is recommended when the device is used in combination therapy or with vaginal or anal electrodes.

Interference 2-pole

Interference 2-pole
The interferential 2-pole waveform is similar to the 4-pole variant, except that the two center frequencies are added within the stimulator itself and applied to the treatment site via a single pair of electrodes. Two separate outputs, ChA and ChB, are available with independent intensity controls.

Otherwise, the settings for treatment time, carrier frequency, base and peak frequency and operation with constant current/voltage are identical.

Russian Currents (VOMIT)

Russian currents
Russian currents, or KOTZ currents, use a 2.5 kHz center frequency that is switched on and off in bursts between 1 and 100 Hz. It is also possible to create an overall envelope with variable on and off times.

The burst frequency can be adjusted from 1 Hz to 100 Hz by touching the "Burst" button. 0-10 Hz is adjustable in 1 Hz increments, 10-50 Hz in 5 Hz increments, and 50-100 Hz in 10 Hz increments.

The medium-frequency bursts used for Russian stimulation create work and rest periods. The stimulation duration (work) is set to 10 seconds. The ratio allows you to adjust the rest period relative to the work period. For example, if the ratio is set to 1:4, the work period is 10 seconds and the rest period is 40 seconds.

TENS pain therapy

TENS pain therapy
Three waveform types (asymmetric, symmetric, and sequential) are available for TENS therapy. Each type is represented graphically on the display.
Frequency: The frequency can be adjusted in steps of 1 Hz from 40 to 250 Hz.
Pulse width: The pulse width can be set from 40 to 400 µs in steps of 5 µs.
Mode selection: TENS treatments can be performed in 3 modes: continuous, burst or swelling.

Diadynamic currents

Diadynamic currents
Type: The desired diadynamic waveform can be selected by touching the Type button. The full range of diadynamic waveforms is available: DF - Diaphase, MF - Monophase, CP - Modulus and Periodic Courts, Cpiso - Modulus and Isodynamic Courts, RS - Syncopated Rhythm, and LP - Modulus and Long Periods.
Polarity : This button allows you to reverse the polarity of the waveforms. This is especially useful in combination therapy, where the ultrasound transducer becomes an electrode, and this button allows you to change the polarity of the waveform relative to the ultrasound transducer. If the Auto-reverse option is selected, the output polarity will automatically reverse halfway through the selected treatment time.

Sinusoidal currents

Sinusoidal currents
Waveforms: Three waveforms (envelopes) are available: trapezoidal, rectangular, and triangular. Slew rate: Slew rates of 2, 5, 10, 20, and 30 per minute are available.

Faradic currents

Faradic currents
Waveforms: Three waveforms are available: rectangular, triangular and trapezoidal - a graphic showing the waveform is displayed on the button.
Slew rate: Slew rates of 2, 5, 10, 20, and 30 per minute are available.

Galvanic currents

Galvanic currents direct current
Polarity: Touching this button sets the electrical polarity of the galvanic current to Pos (+), Neg (-) or Autorev (the polarity is reversed halfway through the selected treatment time).

Ultra-stimulation current according to Trabert

Trabert
The available options are treatment time, polarity, and constant current/constant voltage. These options are set in the same way as for the other current types.

Microcurrents

Microcurrents
Frequency : The frequency can be adjusted from 1 Hz to 1000 Hz in a pop-up window. Polarity: This can be Pos (+), Neg (-), or Autorev (the polarity is reversed halfway through the selected treatment time).

S/D curves

S/D curve
This mode generates rectangular, intermittent galvanic pulses for plotting strength/duration curves. The output from Channel A is only available at constant voltage.

The start pulse width can be set from 100 ms to 1 s, and the end pulse width from 10 µs to 1 ms. This defines the measurement range and thus the start and end points of the S/D graph.

The graph at the bottom of the screen shows the pulse width and the progress indicator, indicating when the pulse occurs. The pulse repetition rate is fixed at 20/min.
